A patient journey map shows every step a patient takes – from first symptoms and diagnosis to treatment, follow-up and ongoing care – with the touchpoints, emotions and pain points at each stage. Use these editable templates to visualize hospital care pathways, pharma patient journeys (from awareness to brand access) and patient experience maps in PowerPoint or Google Slides. For the full hospital flow, start with our patient journey map template; for sales and marketing journeys, see customer journey map templates.
Patient journey mapping is the process of visualizing a patient’s end-to-end experience with a healthcare provider or treatment – stages, touchpoints, emotions and pain points – so care teams and pharma brands can improve outcomes and experience.
Hospital operations and quality teams, patient-experience leads, pharma brand and market-access teams, and healthcare consultants.
A common model uses five stages: Awareness (the patient notices a need for care), Consideration (compares providers or treatments), Access (books and reaches care), Service delivery (the clinical encounter) and Ongoing care (follow-up and long-term engagement). Our hospital and visit-based templates follow this flow; our pharma templates add Diagnosis, Treatment Selection and Brand Access.
The patient journey is the sequence of steps a patient takes across the system; patient experience is how the patient perceives and feels about each of those steps. A patient journey map shows both.
Care-delivery maps typically use Arrival, Assessment, Care and Outcome or Pre-visit, Check-in, Consultation and Follow-up. Pharma maps typically use Awareness & Recognition, Presentation & Diagnosis, Treatment Selection, Brand Selection, Brand Access and Switching & Progression. Both structures are available here.
Yes – each template opens in PowerPoint and Google Slides in 16:9 and 4:3.
Four are common: current-state maps (how care works today), future-state maps (the target experience), day-in-the-life maps (what patients do inside and outside the clinic) and service blueprints (front-stage and back-stage processes behind each step).
